Thank you for choosing our services. We would just like to take a moment to share with you how things work at Family TLC. Our services are confidential and no one at Family TLC will release your name or information about you or your counselling to anyone outside of Family TLC’s office without your informed, voluntary and written consent, except as outlined by law and in the Privacy and Practices Policy below. These exceptions include: 1) a client’s imminent to end their life or to harm others, 2) a member of the vulnerable sector suspected of being at risk or abused, 3) a court order to release a client’s file, 4) the reporting of a professional who has sexually abused a client.
Now that you have booked your first appointment, we consider you a Family TLC client. As you know through the intake process, you were matched with a therapist who works with Family TLC clients. All Family TLC clients have the benefit of a team approach which includes the therapists they are working with who are supervised by Sue Cook.
All professionals who work with Family TLC clients have signed a confidentiality agreement and meet weekly for clinical support. The therapist that you are seeing may bring your case up for discussion anonymously.
In the interest of protecting all Family TLC clients, all and any session notes will not be released for litigation purposes. I, the client, agree by signing this document that I will not issue a summons requiring the person(s) who conducted the initial intake, and/or the counselling, coaching and/or mediation to appear before a judge, arbitrator, or tribunal in proceedings against a family member, spouse or partner to disclose any information obtained from him or her. In the event that a member of the Family TLC team is subpoenaed to court you will be responsible to pay the Family TLC team member their hourly rate for preparation time, time spent at court as well as testifying.
Each counselling session has a set fee. Payment will be collected at the beginning of your appointment with cash, cheque, credit card or debit. Our cancellation policy is simple: we ask that you provide us with at least 2 business days’ notice if you need to cancel or reschedule an appointment to avoid a cancellation fee. With insufficient notice the session fee does apply.
Please Note the average session is 50 minutes in length however, with special request, the time may be extended if scheduling permits and you will be billed accordingly.
We encourage the following for all client relationships with Family TLC:
- A mutually respectful session between clients and their therapists
- An understanding of the importance of counselling and therapy
- The client’s own application of therapy learnings
- An understanding of all information on this website as purely educational and not therapeutic or clinical in nature
- The client’s understanding and responsibility of their own benefit coverage programs
